Commitment Form for Elementary Families Still in Distance Learning

On March 29, 2021, elementary school principals sent a survey to families who so far have decided to stay in distance learning for the rest of the school year.  This survey (or "commitment form") is different than the previous one sent in March, in that it asks families to commit to either keeping their child in distance learning for the rest of 2020-21 or committing to sending the student back to in-person instruction. The survey should take only a minute or two to complete. We are asking families to submit one form for each student they have in elementary school by April 5.
