Screening for COVID-19 Symptoms

 

The California Department of Public Health (CDPH) and Alameda County Public Health Department are no longer requiring families and students to fill out a health screener (e.g., Qualtrics) before school each day.  Instead, we ask that you check your child daily for symptoms indicative of COVID-19, including: 

 

  • Fever (100.0 F or more) or chills 

  • Cough 

  • Shortness of breath or difficulty breathing 

  • Severe headache 

  • New loss of taste or smell 

  • Sore throat  

  • Nausea or vomiting 

  • Diarrhea

  • Runny nose or congestion

  • Fatigue

  • Muscle or body aches

  • Headache

If your child has any of these symptoms, please keep them home from school and fill out the new Symptom Report Form on your school’s website. It takes less than five minutes to fill out the form. Once you submit it, you’ll receive an email with instructions about your child’s isolation dates, local testing sites, and other important information.

Please note that students should also stay home if they:

 

  • Have had a positive test for COVID-19 in the last ten days or
  • Have been exposed to someone with COVID-19 off campus in the last 10 days
